We take the time to create a personalized treatment for each patient and we offer a wide range of services. It is our goal to make your visit with us an extremely pleasant one and to send you home with a beautiful and healthy smile.
Below are our New Patient forms that are required prior to your first visit. Each form can be filled out electronically and once you press "Submit" at the bottom of the page, the form will be sent directly to our office. The Records Release form allows us to contact your previous dentist and retrieve your records - the sooner you fill out the record release form, the better, as it gives us time to acquire your records prior to your first visit in our office.
We kindly ask that you take a moment to fill out the above forms prior to your first visit. However, if you are unable to do so, we can provide paper copies that can be completed when you arrive. Please bring your dental insurance card (if you have dental insurance) as well.
During your initial visit, you will meet our staff and discuss your dental concerns with Dr. Ganley. A comprehensive exam will be performed including a dental charting, periodontal charting, and any necessary x-rays. In most cases, a dental cleaning will be performed at your first visit, unless a deep cleaning is indicated.
